RESOLUTION NO. 2012-015
ACCEPTING IMPROVEMENTS FOR 2008 HAIL DAMAGE REPAIRS TO CITY
BUILDINGS AND APPROVING FINAL PAYMENT
WHEREAS, pursuant to a written contract with the City of Monticello awarded to Dayle Veches
Construction, Inc. of Monticello, Minnesota, the contractor has satisfactorily completed the work
for the hail damage repair work on Monticello city buildings in accordance with the contract;
NOW THEREFORE, BE IT RESOLVED BY THE CITY OF MONTICELL0, MINESOTA
that the work completed under said contract is hereby accepted and approved and that the Mayor
and City Cleric are hereby directed to issue a proper order for the final payment on such contract
subject to receipt of the following:
1) Final invoice and completed W -9;
2) Satisfactory showing that the contractor has complied with the provisions of Minnesota
Statutes 290.92 requiring withholding of state income tax (IC 134);
3) Lien releases showing subcontractors have been paid.
