ACCEPTING WORK
EX'T'ENSION OF MEADOW OAK AVENUE FROM CURRENT TERMINI TO CSAH 18
AND RECONSTRUCTION OF A SEGMENT OF CSAH 18 AND CSAH 39
CITY PROJECT NO. 2005-05C AND 2004 -01C
WHEREAS, pursuant to a written contract with the City of Monticello awarded to R.L. Larson of St.
Cloud, Minnesota the contractor has satisfactorily completed improvements for the extension of
Meadow Oak Avenue from its current termini to CSAH 18 and the reconstruction of a segment of
CSAH 18 and CSAH 39 for the CSAH 1811 -94 Interchange Project
NOW THEREFORE, BE IT RESOL TEED BY THE CITY OF MONTICELLO, MINESOTA that
the work completed under said contract is hereby accepted and approved and that the Mayor and City
Clerk are hereby directed to issue a proper order for the final payment on such contract subject to
receipt of the following: 1) Satisfactory showing that the contractor has complied with the provisions
of Minnesota Statutes 290.92 requiring withholding state income tax; 2) Evidence in the form of an
affidavit that all claims against the contractor by reasons of the contract have been fully paid or
satisfactorily secured; 3) Consent of Surety to Final Payment certification from the contractor's
surety and 4) Two year maintenance bond to start from the date of final acceptance of the project by
the City Council.
